A multidisciplinary team of a doctor, a nurse, a care assistant will travel daily to ensure the continuity of care and will be in direct and permanent contact with your treating or family doctor to monitor your state of health and the good progress of your treatment. 

In the event of a severe complication, an ambulance can be used to transfer you to our emergency room or to a healthcare facility.

How can one have access to this service?

Life I Hospital at Home service will be implemented at the request of your doctor or following a referral by the patient or next of kin.

The doctor and the nurse coordinator will proceed to a medical assessment at home for a potential admission in Life I Hospital at Home service. Your state of health and the care to be provided must meet predefined criterias derived from international recommendations

Your home must be suitable for your hospitalisation. A pre-home visit is thus required.

The team reserves the right to refuse hospitalisation if the criteria are not met.

If you are eligible for Life I Hospital at Home care, the multidisciplinary team will come to arrange your room and you will have the same services and the same equipment as in a healthcare facility.

The admission process

Step 1.

Your doctor refers you to Life I Hospital at Home or you/your relatives agree with your doctor that your care and treatment will be continued under Life I Hospital at Home services.

 

Step 2.

Life I Hospital at Home team assesses the request and contacts you for an evaluation of your home place and treatment plan.

Step 3.

If you meet the criteria for admission and subject to your consent, your personal information, your medical file, and your inpatient insurance coverage (if applicable) are requested.

Step 4.

Your room is fitted out, if necessary, by the mobile team.

All the necessary equipment for your treatment is supplied and installed. Prescribed medications, as availability permits, are also provided. Life | Hospital at Home team conducts regular visits and implements treatment tailored to the patient’s needs. The team can stay in contact with your treating or family doctor, if needed/requested.

Life I Hospital at Home palliative care aims to provide comfort care, by offering medical and nursing treatment to best control symptoms such as pain, agitation, anxiety, nausea, and vomiting, and so as early as possible, including the psychological well-being of the patient and the family.
